Required Documentation

Active Duty Service Member

US Armed Forces active duty service members who have served greater than 90 days may be eligible for bonus compensation if they meet the eligibility criteria. Required documentation includes:

  • Military records showing dates when serving in Afghanistan and/or Iraq or anywhere else in the world.
  • Proof of current Ohio residency.
  • Proof of Ohio residency at start of military service.
  • The following documents may be used to establish Ohio residency:
    • Leave and Earnings Statement
    • Ohio driver’s license
    • Ohio State Income Tax Return
    • Voter registration records

Veterans

Former members of the US Armed Forces who served greater than 90 days may be eligible for bonus compensation if they meet the eligibility criteria. Required documentation includes:

  • DD-214, Member Copy 2 or 4 (will provide home of record at start of military service, dates when serving in Afghanistan and/or Iraq or anywhere else in the world, and character of service at discharge.)
  • Proof of Ohio residency at start of military service if DD-214 shows other than Ohio as "home of record".
  • Proof of current Ohio residency
  • The following documents may be used to establish Ohio residency:
    • Ohio driver’s license
    • Ohio State Income Tax Return
    • Voter registration records

Medically Discharged or Medically Retired From Service Due to Combat-Related Disabilities Sustained During Afghanistan or Iraq Service

Medically discharged or retired veterans who meet all eligibility criteria above may be eligible for an in-theater bonus of $1,000 (regardless of time served in-theater), and may be eligible for non-theater service at $50 per month, excluding training time. The maximum benefit award is $500 for non-theater service. A medically discharged or medically retired veteran may be eligible for a total maximum benefit of $1,500. Required documentation includes:

  • DD-214, Member Copy 2 or 4 (will provide home of record at start of military service, dates when serving in Afghanistan and/or Iraq or anywhere else in the world, and character of service at discharge.)
  • Proof of Ohio residency at start of military service.
  • Proof of current Ohio residency
  • The following documents may be used to establish Ohio residency:
    • Leave and Earnings Statement
    • Ohio driver’s license
    • Ohio State Income Tax Return
    • Voter registration records
